An evolution of the left anterior small thoracotomy proposed by Calafiore in the early ‘90s is presented. LITA and RA are harvested and joined in a composite Y-graft. Medtronic Octopus® and Starfish® NS are used to expose and stabilize the coronary arteries. Distal anastomoses are completed using conventional instruments.
