OPCAB, 29 YEARS OF FOLLOW UP

OBJECTIVE:
Off -Pump coronary revascularization (OPCAB) has been adopted by many surgeons as an alternative to conventional on -Pump coronary artery bypass surgery (CABG) However ,there is very little long term follow up The goal of this study was to evaluate the long term outcome of patient who have been treated with this technique.
METHODS:
From 1978 to 2007 we performed coronary bypass surgery on beating heart in a series of 1728 patients, this represents and overall applicability of sixty four percent, considering a total off 2717 revascularization procedures performed during this time. There were 1210 (70 %) males with age from 11 to 86 (Mean 64±12). Mean graphs per patients was 2.1 and the Internal Mammary Artery was used in 70 % of the cases
RESULTS:
The Mortality for entire OPCAB series was 46 patients(2,7 %). The mean operative time was (261 minutes ± 59). Mean hospital stay 5.2 days, and the cost reduction compared to conventional CABG is 34 %. The long term survival at 29 years was 25 % vs. 10 % (KM) in the similar group of patients than ON PUMP (p=0.03).
CONCLUSIONS:
The long Term results of the initial series of OPCAB confirm these operation as alternative treatment in patients with coronary disease. The expanding of the OPCAB depends of the evolution of the technologies and a intensive education and training process of the cardiac surgeons.
